The most significant side effect of testosterone is its androgenic effect. Especially when used illegally (doping in sports) in women, it leads to virilization (masculinization), acne vulgaris, androgenetic alopecia, hirsutism, and voice changes such as a hoarse or raspy voice (caution advised for women in singing or speaking professions!).

Prolonged use of testosterone leads to water retention and edema. Regarding its effect on the prostate in men, it should be noted that androgens such as testosterone can promote the growth of a malignant prostate tumor.
